Care Coordinator

Remote (United States)

We’re looking for a Care Coordinator to join our Care Coordination Team, within the Service Operations Department.


In this role, you will work closely with our providers and patients through many stages of their Thirty Madison journey. The role is both patient and provider-facing, from helping patients receive medical records, to assisting providers in coordinating treatment plans. You’ll partner with and support the Customer Experience team, our medical network, and patients across all of our brands. You must be comfortable speaking with others across various channels (primarily email/message) compassionately and professionally.


This position will report to the Medical Operations Manager - Care Coordination and work cross-functionally within the Service Operations team in the constant pursuit of a better patient experience.

What you get to do every day

  • Manage patient/provider messaging and order fulfillment queues
  • Provide administrative and communication support for our provider network
  • Coordinate the treatment and community resources available to patients
  • Ensure that our patients’ needs and preferences for health services and information-sharing across providers, functions, and brands are met
  • Coordinate the external provider referral and diagnostics processes
  • Create and send medical records and doctor notes to patients who request them
  • Other ad-hoc, medical-operations related tasks


What you bring to the role

  • At least 1 year of experience in a customer-facing or patient-facing role
  • Experience in clinics, hospitals, medical offices, or similar fields is a major plus!
  • Excellent communication skills, knowledge of general clerical duties, and cultural competency for communicating with people of various backgrounds
  • Aptitude and enthusiasm to learn the ins and outs of a fast-moving startup in the healthcare world
  • Superb judgment, the ability to think quickly on your feet, and with deep empathy
